Your Role: Home Health Physical Therapist (PT)

As a **Physical Therapist in Home Health**, you will provide **skilled rehab services** tailored to each patient’s needs. Your focus will include **fall prevention, strength building, mobility training, and functional independence** for **senior and geriatric patients**.

Key Responsibilities:

• Conduct **in-home physical therapy evaluations and reassessments**
• Develop and implement **personalized rehabilitation care plans**
• Improve **strength, balance, mobility, and functional outcomes**
• Provide therapy in **home health, assisted living, and community care settings**
• Educate patients and families on **fall prevention and safe mobility techniques**
• Document visits using **WellSky (Kinnser) EMR**
• Collaborate with **interdisciplinary home health teams**

Qualifications

• **Licensed Physical Therapist (PT) – California (Required)**
• **CPR Certification (AHA approved)**
• Minimum **1 year of clinical experience** (Home Health preferred)
• Experience in **senior care, geriatrics, rehab, or community-based care** preferred
• Strong **clinical, communication, and patient education skills**
• Valid **driver’s license, reliable transportation, and auto insurance**
