Physician Access Nurse(RN)-Mt Sinai 42nd St Physician Services- FT Days 11a-7p M-F

The Clinical Nurse for Physician Access Services is a Registered Professional Nurse with broad knowledge and skills who complete intake and triages to clinical program or services using a decision algorithm for designated patient populations. This position coordinates and facilitates patient referrals of affiliated practices into the Mount Sinai Health System. This nurse will be working at Mount Sinais corporate office near Grand Central. The Clinical Nurse for Physician Access Services works with a dynamic group of nurses and coordinators to help patients gain access to healthcare across the entire health system.
Patient Care

• Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards.
• Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
• Serves as the point of contact for physician referrals and obtains necessary intake information to initiate the patients referral into Mount Sinai Health System.
• Site visits to maintain key partnerships with existing and potential physician practices, hospital affiliates, or any other identified referral base.
• Advocates for patients, families, and referring physicians to promote overall satisfaction with accessing and utilizing Mount Sinai Health System.
• Communicates information between patient, referrer, family, insurance company as indicated.
• Assists the Patient Transfer Service with coordinating hospital transfers into the Mount Sinai Health System using an established decision algorithm.
• Promptly answers and screens inquiries with strict adherence to confidentially agreements, policies and procedures.
• Performs insurance verification and obtains authorization as needed.
• Integrates relevant assessment and intervention skills in the delivery of patient care coordination.

Patient Experience

• Initiates service recovery for patients and family members who have concerns and escalates to Supervisor, Manager/Clinical Manager/Director as needed.
• Demonstrates caring, respect, compassion, empathy and active listening through dialogue and actions.
• Approaches patients, families and other members of the health care team in a professional, respectful manner.

Quality and Safety

• Responsible for the complete and accurate registration of all patients in an expeditious and positive manner.
• Documents all patient encounters and enters all data into referral tracking system.
• Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol.
• Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives.

Operations

• Performs nurse duties as assigned.
• Escalates material or staffing resource deficits to Management team.
• Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources.
• Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care.
• Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards.
• Work-schedule that include rotating weekends and holidays.
• Perform other duties as assigned by Manager and Director.

Education Requirements
BSN preferred.
Experience Requirements
1-2 years acute care medical/surgical experience. Desire to work in a behind the scenes setting and use your clinical skills is essential.

The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $64.85 - $64.85 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
