<p><span data-contrast="auto">As the Implementation Manager for Evergreen Connected Care (ECC), you own the day-to-day execution of practice onboarding and the ongoing operating relationships with our partner practices. Connected Care is delivered as a white-labeled extension of the practice and its providers; you are the person who makes each launch happen on time and keeps the relationship running well afterward.</span><span data-ccp-props="{}"> </span></p>
<p><span data-contrast="auto">You run the standard onboarding sequence at each practice - kickoff, implementation and breakout meetings, final all-hands, and the controlled outreach start - and you hold the timeline. You build and refine the meeting materials, facilitate sessions alongside the Program and Clinical Director or on your own, track every open item to closure, and make sure the right people know what is expected of them and by when.</span><span data-ccp-props="{}"> </span></p>
