Care Manager - Home Care - Warragul

Are you passionate about helping older Australians remain independent in their own homes? Healthcare Australia is seeking a people-focused Home Care Coordinator to support clients across the surrounding Warragul area.

In this rewarding client-facing role, you'll build strong relationships with clients and their families, coordinate services that meet individual needs, and act as a trusted point of contact throughout their Home Care journey. You'll spend time meeting with clients in their homes, conducting assessments of their needs, developing care plans, and working closely with internal teams to ensure exceptional service delivery. In this role you will have variety, working across the office and driving out to meet clients in their homes. No two days are the same.

A typical day could look like

• Meeting clients and their families to understand their goals, preferences, and individual needs.
• Collaborating with Registered Nurses and Allied Health professionals to coordinate care for clients with complex needs.
• Guiding potential clients through the enquiry and onboarding process with empathy and clarity, helping them make informed choices.

Let's talk about you!

A successful candidate will demonstrate a strong understanding of Aged Care and/or other health and community services. You will bring experience from working in customer focused environments and have excellent time management and prioritisation skills, exceptional communication skills, both written and verbal - imperative to working in this virtual role. You will be able to demonstrate a willingness to learn and adapt to changing environments/ markets. Candidates with a background in nursing (EN or RN) are highly desirable.
