The Registered Dietitian (RD) is responsible for planning, organizing, implementing, and evaluating nutritional care and foodservice programs for residents and patients within skilled nursing, rehabilitation, post-acute, long-term care, and healthcare settings. The RD serves as an integral member of the interdisciplinary team, ensuring optimal nutritional status, regulatory compliance, quality outcomes, and resident satisfaction. The position supports resident-centered care while working collaboratively with physicians, nursing staff, rehabilitation services, speech therapy, foodservice management, and families.
Essential Duties and Responsibilities
Clinical Nutrition Management
- Complete comprehensive nutrition assessments for all residents/patients according to facility policy and regulatory requirements.
- Develop individualized nutrition care plans based on medical diagnosis, nutritional risk, laboratory values, medications, and resident preferences.
- Monitor and evaluate residents at nutritional risk, including:
- o Significant weight changes
- o Malnutrition
- o Dehydration
- o Pressure injuries
- o Diabetes
- o Renal disease
- o Cardiac conditions
- o Nutrition support therapies
- Provide medical nutrition therapy (MNT) and ongoing nutrition interventions.
- Review and update nutrition care plans as clinical conditions change.
- Document assessments, recommendations, interventions, and outcomes in the electronic medical record.
- Participate in admission, quarterly, annual, and significant change assessments.
Regulatory & Compliance Responsibilities
- Ensure compliance with all applicable CMS, state, federal, and accrediting agency regulations.
- Maintain current knowledge of long-term care regulations and best practices.
- Assist facility leadership during state surveys and regulatory audits.
- Monitor and support compliance with food safety and sanitation standards.
- Participate in Quality Assurance and Performance Improvement (QAPI) activities.
- Assist with development and revision of dietary policies and procedures.
Interdisciplinary Team Collaboration
- Participate in care conferences and interdisciplinary team meetings.
- Collaborate with physicians, nurse practitioners, nursing leadership, rehabilitation staff, speech-language pathologists, social services, and foodservice personnel.
- Communicate nutrition-related recommendations and interventions.
- Support resident-centered decisions regarding diet liberalization and quality of life initiatives.
- Educate caregivers, nursing staff, residents, and families regarding nutrition-related concerns and interventions.
Food Service & Menu Management
- Review and approve menus to ensure nutritional adequacy and regulatory compliance.
- Collaborate with culinary and dining service teams to enhance:
- o Resident satisfaction
- o Menu variety
- o Therapeutic diets
- o Cultural preferences
- o Seasonal offerings
- Conduct routine kitchen and dining room audits.
- Monitor food quality, meal service, sanitation, and customer satisfaction.
- Recommend supplements, fortified foods, texture modifications, and therapeutic diet adjustments as needed.
- Support implementation of person-centered dining programs.
Quality Improvement & Leadership
- Participate in QAPI initiatives focused on:
- o Weight management
- o Malnutrition prevention
- o Hydration
- o Wound healing
- o Resident satisfaction
- Analyze nutrition-related clinical outcomes and trends.
- Provide recommendations to improve clinical and financial performance.
- Support dining enhancement programs and hospitality-focused initiatives.
- Serve as a nutrition resource for facility leadership and staff.

Requirements
Qualifications
Education
- Bachelor's or Master's Degree in Nutrition, Dietetics, Food and Nutrition, or related field from an accredited institution.
Credentials
- Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR).
- Current state licensure/certification where required.
- Certified Dietary Manager (CDM) knowledge preferred but not required.
Knowledge, Skills and Abilities
- Strong understanding of CMS and long-term care regulations.
- Knowledge of medical nutrition therapy and therapeutic diets.
- Experience with electronic health records (EHR).
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to manage multiple priorities independently.
- Knowledge of texture-modified diets and dysphagia management.
- Understanding of infection prevention, food safety, and sanitation requirements.
- Strong collaboration and leadership skills.
Physical Requirements
- Ability to visit patient rooms, kitchens, dining areas, and clinical units.
- Ability to sit, stand, walk, bend, and lift up to 25 pounds occasionally.
- Ability to work in both clinical and foodservice environments.
