Per Diem/Day Shift

The Community Health Worker (CHW) is a trusted community member who acts as a liaison between a community and health/social service systems. The CHW is knowledgeable about community resources and uses their lived experience to build trusting relationships with healthcare providers, community members, patients and community partner organizations.

Required Qualifications

• Coursework/Training: Complete 80+ hours training as Community Health Workers. Approved by the Oregon Health CHW Training curriculums.
• 1-3 years relevant community experience.
