Southern Health Partners is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide chronic care services at the Madison County Jail in Alabama.

💚 What You’ll Do

As a Chronic Care LPN, you’ll play an important role in helping patients manage ongoing health conditions and receive consistent, quality care.

Provide nursing care to patients with chronic and ongoing healthcare needs
Monitor patients' conditions and identify changes that may require additional evaluation
Administer medications and treatments according to provider orders
Perform nursing assessments and document patient care accurately
Assist with chronic disease management and follow-up care
Communicate patient needs and changes in condition to the appropriate healthcare providers
Educate patients on medications, treatment plans, and healthy practices when appropriate
Maintain accurate medical records and follow established nursing protocols
Collaborate with the healthcare team to support continuity and quality of care

Qualifications🩺 Qualifications

Current, unrestricted LPN license in Alabama
Current CPR/BLS certification
Strong clinical assessment, medication administration, and documentation skills
Ability to work independently while collaborating with the healthcare team
Compassionate, professional approach to patient care
Previous correctional, chronic care, or behavioral health experience is helpful but not required
