About The Role

DescriptionMidland Care Connection

Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.

CNA/Facility Aide Role

This CNA/Facility Aide position provides compassionate, hands-on care to residents in a long-term care facility, helping them maintain their health, safety, comfort, and dignity. Responsibilities include assisting residents with activities of daily living (ADLs), such as bathing, dressing, grooming, toileting, eating, and mobility. The role also involves helping residents reposition safely and supporting daily routines to promote independence whenever possible.

CNAs/Facility Aides observe residents for changes in their physical, emotional, or mental condition and promptly report concerns to the nursing team. They assist with maintaining a clean, safe, and supportive environment, provide companionship and emotional support, and work collaboratively with nurses and other healthcare professionals to ensure high-quality resident care.
