ABOUT THE ROLE We’re searching for a dynamic, hands-on Clinical Care Coordinator to lead health and wellness operations in our Assisted Living and Memory Care units. You’ll be more than a nurse — you’ll be a mentor, problem-solver, and advocate, ensuring every resident receives personalized, compassionate care while empowering your team to deliver their very best. WHAT YOU’LL DO Ensure top-quality care on every shift (and step in to assist as needed). Lead, coach, and inspire nursing staff, medication techs, CMAs, and caregivers. Train team members with engaging, real-world learning. Maintain accurate charts and documentation. Assess, plan, and update individualized Clinical Care Plans. Spot and address health concerns early to help residents stay in their home longer. Oversee medication management, training, and audits. Share on-call and Manager-on-Duty responsibilities. Assist with onboarding new team members.
