Design and implement PT plan of care. Provide evidence-based interventions. Document all encounters in the electronic medical record. Complies with departmental policies/practices. Maintains patient safety, provides patient/family/caregiver education. Works cohesively in a team setting.

Minimum Education: Graduate of an accredited PT program Minimum Experience: 2-3 years working in an outpatient department preferred License/Certification: NJ PT License
