KENOSHA HUMAN DEVELOPMENT SERVICES, INC. 3536 52nd Street Kenosha, WI 53144 Division/Department: Sunrise Clinical Services Location: Main Office Job Title: Advanced Practice Nurse Prescriber Reports To: Director of Crisis and Stabilization Services / Director of Community Recovery Services Type of Position: Full-time FLSA Status: Non-Exempt Schedule: Monday - Friday, 40 hours per week POSITION SUMMARY This position provides outpatient medication management and brief therapy services to consumers in the Sunrise Clinic and the KARE Center. The position requires 15 hours of time be dedicated to residents at the KARE Center. RESPONSIBILITIES 1. Assess and diagnose consumers. Coordinate with program manager or assigned staff to identify consumers for assessment an diagnosis. 2. Complete required assessment, progress notes, and discharge as required by DHS 35. 3. Prescribe and renew medications as deemed appropriate. 4. Provide medication monitoring. 5. Contact resident's current provider of psychiatric services and collaborate to meet the resident's needs while at KARE Center. 6. Provide standing orders for all KARE Center admits. Review medication orders as appropriate and communicate medication orders with LPN for quality of care of KARE Center residents. 7. Make referrals for other services and community resources as necessary. 8. Coordinate with clinic office and building staff to help ensure accurate claims processing and billing. 9. Attend clinical consultation with multi-disciplinary teams. 10. Consult with staff psychiatrist as required per Wisconsin State Statutes. 11. Medication administration of medications (oral and injection) in office or community as necessary 12. Maintain daily medication administration charts to ensure accuracy of medication administration, office pick up and home delivery 13. Review and coordinate client signature on medication informed consent forms annually and as new medication is prescribed. 14. Provide support to CSP nurses as needed. PROFESSIONALISM 1. Conduct oneself in a professional manner a. Maintain the confidentiality and other rights of participants, families and staff and follow HIPPA Guidelines for all verbal and written communication b. Problem-solve difficult situations in a timely manner c. Manage difficult or emotional consumer situations by responding promptly to needs, requests for assistance and services d. Give, receive and act upon feedback from supervisors and co-workers in an appropriate manner e. Maintain professional boundaries with consumers and co-workers at all times f. Attend training as requested by supervisors g. Other tasks as assigned by supervisors QUALIFICATIONS: A Master's Degree (or higher) in Nursing with active Advanced Practice Nurse Prescriber credential required. 1. Actively enrolled in courses and working on Psychiatric Certificate as a minimum requirement 2. Able to work with children and adults 3. Experience in evaluating and treating adults with serious and persistent mental illness in either an outpatient or inpatient facility 4. Ability to effectively communicate, both written and verbally, in the English language 5. Must have reliable transportation and valid driver’s license with good driving record 6. Must carry own auto insurance with at least 100,000/300,000/100,000 coverage 7. Ability to maintain records and document case notes daily 8. Ability to give and accept feedback as a part of a team 9. Ability to traverse within the workspace and community frequently 10. Be willing to learn and work with the treatment philosophies 11.
