About The Role

Duties

  • Consistently follows verbal and written assignments to complete daily tasks
  • Assists residents with both short and long distance ambulation, transfer exercises
  • Provides/oversees PROM and AROM programs for selected residents
  • Assists residents to perform exercises utilizing parallel bars, finger ladder, stairs,

pulleys, etc...

  • Directs group and individual exercise programs in licensed units
  • Assists residents with occupational rehabilitation exercises
  • Assists residents with oral exercises
  • Proficiently measures and documents vital signs including blood pressure, pulse, respiratory rate, temperature and weight as directed
  • Documents resident response to rehabilitation programs
  • Encourages maintenance of optimum nutritional standards by: maintaining an environment conducive to enjoyable dining; focusing attention on individual residents, completing plate preparation(cutting food, opening straws) assisting resident as needed with reminders or cueing utilizing proper feeding techniques to increase independence
  • Provides all care in a manner protecting residents personal dignity and enhances the resident’s personal choice and individuality
  • Collects urine and stool samples following policy and procedure
  • Completes oral hygiene, and denture care following resident care guide and established policies and procedures
  • Assists residents to maintain continence throughout the course of the shift by

providing reminders, cues or assist as necessary.

  • Completes skin care after episodes of incontinence following resident=s care guide and skin care policy and procedure
  • Completes foley care following policy and procedure
  • Repositions resident throughout shift per resident care guide and policy and procedure
  • Completes AM/PM care (shaving, back rub, assistive devices, clothing choices, etc...) following specific resident requests in care guide and policy and procedure
  • Assists resident with complete bath/shower(hair care, foot care, nail care, shaving, dressing, etc...) as needed following resident requests in care guide and policy and
  • procedure
  • *Completes 1/12th, Customer Service, and CPR certification
  • *Participates in DeKalb Area Retirement Center Staff Development Programs.
  • Consistently follows hand washing policy and procedure
  • *Reports all changes in/observations of residents condition to licensed staff
  • *Reports all changes in behavior or functional abilities, as well as, signs of accident, injury or
  • illness.
  • *Guides the resident in maintaining his/her schedule of daily activities, while maintaining the resident’s sense of independence.
  • *Individualizes care and assistance provided to each resident dependent on level of functioning.
  • *Helps the resident focus energy into activities meaningful to the resident.
  • *Ensures resident privacy, dignity and confidentiality.
  • *Consistently checks residents environment after completing care to be sure environment
  • is left neat and orderly
  • *Provides all care in a manner protecting residents personal dignity and enhances the resident’s personal choice and individuality
  • *Communicates positively and appropriately with family and visitors.
  • Establishes and maintains open lines of communication with coworkers and immediate supervisors
  • Develops and maintains professional positive and supportive working relationships with coworkers in rehabilitation, nursing, and other departments for the mutual goal of quality service to residents
  • Recognizes that resident needs may necessitate working as a staff CNA and accepts

reassignment in a cooperative manner

  • Courteous and modifies tone and volume of voice when communicating, does not approach coworkers in an aggressive/confrontational manner
  • Refers concerns and complaints to Restorative Nurse
  • Serves as a productive team member and as a resource person by assisting other CNA staff with documentation and rehabilitation programs
  • *Assures that all documentation applicable to rehabilitation CNA on the assigned shift is accurate, timely and thorough.
  • *Flexible within the program structure and makes adjustments to meet the residents= needs.
  • *Responds calmly in emergency situations and assists as directed by licensed staff
  • *Participates with new team members by serving as a resource to new staff.
  • *Recognizes potential customer service problems and seeks assistance from immediate supervisor
  • *Maintains confidentiality of all work-related concerns
  • *Consistently adheres to organizational requirements for hygiene, appearance, dress as stipulated in the Employee Handbook/Departmental Policies.
  • *All other duties assigned

Requirements

Minimum Education Requirements

  • Requires knowledge, skill and mental development equivalent
  • to the completion of two years of high school.
  • CNA certification
  • Certification as a Rehabilitation Aide preferred

Experience

  • 1 year CNA experience in Long Term Care
  • 6 month CNA experience within organization

Other

  • Ability to work in a health care and CCRC environment.
  • Knowledge and practice of safe lifting and back care techniques.
  • Ability to stand and walk a minimum of 90% of work day.
  • Ability to see and hear or use prosthetics adequately so that the tasks of this position
  • can be fully met, to include the use of supplies and equipment.
  • Ability to bend, turn, reach over head and below knees throughout work day.
  • Ability to speak, read, write and understand English.
  • Ability to receive and follow directions.
  • Comply with all DeKalb Area Retirement Center rules, policies and procedures.
  • Ability to attend meetings and inservice presentations as required.
  • Ability to work in and maintain a flexible, resident-centered environment.
  • Ability to recognize and respond to resident needs.
  • Ability to assume a supportive role in resident initiated activities.
  • All resident care and assigned leadership tasks as described under duties and responsibilities,
  • under the direction and supervision of the staff nurse, unit manager and/or Director of Nursing.
