Job Summary

• Responsible for the entire process of sterile compounding; assuring quality assurance goals are met by
• the organization and maintaining compliance with State and Federal laws and standards related to sterile
• and hazardous compounding. Develops and performs competencies for staff members in the area of sterile
• and hazardous medication compounding; maintain appropriate records related to sterile compounding and
• monitor quality assurance program for sterile compounding Performs as a staff pharmacist within the sterile
• compounding environment. Responsible for medication preparation and distribution; evaluating, inputting
• and verification of new medication orders; assisting providers and other health care professionals by
• serving as a medication information resource; and managing pharmacy inventory.

Minimum Education and Training

• Bachelor's Degree from an accredited College of Pharmacy
• Kentucky and/or Indiana Board of Pharmacy license in good standing as applicable to practice site
• Two (2) years of hospital experience in sterile compounding (as a pharmacist or technician) required
• Active Board Certification in Compounded Sterile Preparations Pharmacy preferred
