About The Role
La Plateforme Inclusion Sociale 78 de la Fondation Falret, pour son Equipe Mobile Passerelle 78 Nord, basée à AUBERGENVILLE, recrute un.e Travailleur Social ( Assistant social, éducateur ou CESF). Il s'agit de prendre en charge en VAD ( évaluation médico-sociale puis accompagnement) des ménages repérés comme ayant de potentiels troubles psychiques. les bénéficiaires sont accompagnés par un binôme travailleur social + psychologue ou IDE. Vous êtes dynamique et aimez travailler en équipe pluridisciplinaire, rejoignez nous !! CDI Temps plein - CC66 - TR - Permis B obligatoire
