About The Role

The FEP Case Manager serves as a primary point of contact for W-2 participants, guiding families through goal setting, barrier removal, and employment planning. This role uses assessment data to determine appropriate service tracks and develops comprehensive strategies that promote self-sufficiency through coordinated support and workforce development.

  • Conduct initial and ongoing assessments to determine participant needs, strengths, and barriers to employment.
  • Assign participants to appropriate W-2 service tracks based on eligibility and assessment outcomes.
  • Collaborate with families to establish employment and self-sufficiency goals, prioritizing barrier removal and skill development.
  • Develop and implement wraparound strategies including job readiness, career planning, budgeting, and skills training.
  • Coordinate activities outlined in the W-2 Individual Employment Plan (IEP) with those of external support providers.
  • Monitor participant progress, update service plans, and adjust strategies as needed to ensure goal attainment.
  • Maintain accurate and timely documentation in compliance with W-2 program standards.
  • Connect families with community resources and services that support workforce participation.
  • Ensure all interactions and documentation comply with applicable laws and program regulations.

Required

  • Associate’s degree in human services, Social Work, or related field.
  • Minimum of 2 years of experience in case management, employment services, or social services.
  • Strong organizational and communication skills.
  • Ability to work collaboratively with diverse populations and service providers.
  • Proficiency in Microsoft Office and case management software.

Preferred

  • Bachelor’s degree in a related field.
  • Experience working with W-2 or similar employment-focused programs.
  • Familiarity with local workforce development and public assistance programs.
  • Bilingual (Spanish or Hmong) a plus.
