7:30 AM-4 PM Scheduled Daily Hours

8.5 HOURS Hours Per Pay Period

40 Pay Rate/Range

$157,000+ per year

Job Summary: A well rounded pharmacist who will be expected to function as an independent, frontline healthcare provider by interacting with patients, pharmacy staff, and members of multidisciplinary healthcare teams. Safely and accurately treat patients by critically reviewing clinical status and available therapeutic options. Participate in multidisciplinary rounds, monitor drug therapy, and intervene when clinically necessary by modifying therapy or consulting with prescribers to recommend alternatives. Identify performance improvement opportunities and work on projects. Be able to cover various areas and shifts including unit based activities, IV compounding, and clinical monitoring. Prepare intravenous admixtures and oversee the pharmacy technicians to compound, package, and label medications to be dispensed. Assist in the development and implementation of new inpatient and ambulatory care pharmacy programs.

Essential Job Qualifications

  1. Experience: 2-3 years hospital pharmacy experience and/or PGY-1 residency preferred.
  2. Training/Certification: Pharmacotherapy or other relevant Board Certification preferred or willing to obtain within 1 year of eligibility.
  3. Licensure: New York State Pharmacist license

C. JOB SPECIFIC COMPETENCIES

Demonstrates the required job specific technical/professional knowledge, skills and abilities. *

  1. Demonstrates sound judgement when providing safe, effective, and compassionate care for patients including solving patient-specific medication related problems, IV and non-sterile preparation, and various aspects of the medication use process.
  2. Participate in multidisciplinary rounds/conversations and regularly consult with prescribers concerning best treatment options.
  3. Takes responsibility for patients who require therapeutic monitoring of medications including ordering appropriate labs, adjusting medication doses, and notifying appropriate healthcare providers as needed.
  4. Documents all required activities in a timely manner.
  5. Identifies and engages in organizational and department quality improvement activities to improve patient care, medication use process, and pharmacy operation.
