About The Role

Join UChicago Medicine Ingalls Memorial Hospital, as a Medical Social Worker, Registry in our Care Coordination department.

The Medical Social Worker at UChicago Medicine Ingalls Memorial Hospital provides clinical social work services to patients and their families through psychosocial assessments, counseling and therapeutic interventions and discharge planning services. Coordinates and facilitates a multi-disciplinary discharge plan for patients with complex social needs. Collaborate with patients, families, healthcare providers and community resources to address barriers to care and support safe transitions across the continuum of care.

Essential Job Functions

  • Psychosocial Assessments and Interventions: Collects, interprets and assesses information regarding patient’s psychosocial level of functioning and social support systems. Documents all needed items in patient record within defined timelines.
  • Complex Discharge Planning: participates in discharge planning process for patients with complex discharge needs.
  • Supports program and hospital goals for discharge planning outcomes.
  • Ensures timely, stable community resources and follow up appointments for discharge to establish continuity of care and prevent readmissions.
  • Communicate with patients, families, care partners, and community resources to ensure handoffs.
  • Provides interventions/resources for: child abuse, domestic violence, guardianship, adoption, sexual assault, advance directives, adult/elder abuse, substance abuse, child protective services and other legal/financial services as needed.
  • Serves as a resource and support to hospital staff, community liaisons, and social service agencies on available resources and eligibility.
  • Actively participates in and, when needed, interdisciplinary rounds and team conference, communications directly with team members, patients, and families to facilitate safe discharges and establish expectations and discharge needs.
  • Document all needed items in patient records within defined timelines.

Required Qualifications

  • Bachelor’s degree in social service with SW experience in acute hospital setting
  • Master’s degree in social service with SW experience in acute hospital setting
  • LSW or LCSW (Illinois)
