Job Summary: The Nurse Care Manager (RN) is an integral part of the patient-centered health home model. This RN Case Management role works collaboratively with the patient, the patient’s family, providers, other health plan divisions, payers, and others to develop and implement a treatment plan that supports self-management of chronic conditions.
