Home Health LVN – Overnight Shift (11 PM–7 AM)

Responsibilities

•  Provide direct patient care in accordance with individualized care plans.
•  Administer medications and treatments as prescribed by the physician.
•  Monitor patients' conditions and report any changes to the healthcare team.
•  Assist with daily living activities, including personal hygiene and mobility support.
•  Educate patients and their families on health management and wellness strategies.
•  Maintain accurate electronic documentation of patient progress and interventions.
•  Participate in ongoing training and professional development opportunities.
•  May be required to travel up to a 30 mile radius.

Qualifications

•  Current LVN license in the state of California.
•  Experience in Nursing
•  Experience with Tracheostomies, G-tubes, and Suprapubic Catheters is preferred
•  Ability to provide critical care for patients with complex needs.
