Customer Care Manager - Home Care

As a Care Coordinator, you'll manage a portfolio of Home Care clients and be their trusted point of contact throughout their care journey.

We are currently looking for people who live near and can service clients in either the Mornington Peninsular, SE Suburbs or Eastern Suburbs . You need to be comfortable travelling to the CBD on office days .

You'll conduct client assessments, develop personalised care plans, coordinate services, monitor outcomes, and work closely with internal teams to ensure every client receives exceptional care.

This is a varied role that combines customer service, care coordination, relationship management and case management. Other day to day activities include:

• Conducting client assessments and develop personalised care plans
• Coordinating services that support client goals and independence
• Building strong relationships with clients, families and referral partners
• Monitoring service delivery and conduct regular care reviews
• Managing Home Care Package budgets and funding
• Supporting client onboarding and respond to customer enquiries or concerns

As a Care Coordinator, you'll enjoy building relationships and making a genuine difference in people's lives.  Experience coordinating services, managing a caseload or supporting clients is ideal, or relevant experience across the Home Care, aged care, disability or broader community services industry.  

Experience with the Support at Home program is highly regarded but not essential.  You will need a full drivers' license and a reliable motor vehicle car to attend client visits at home.
