Direct Care & Daily Living

• Personal Care Assistance: Support clients with daily routines, including bathing, dressing, grooming, and toileting, always prioritizing their comfort and privacy.
• Mobility Support: Assist individuals with physical disabilities in safe transfers (e.g., bed to wheelchair) and navigating their environment to prevent falls.
• Meal Preparation: Plan and prepare nutritious meals that align with specific dietary restrictions or sensory preferences, assisting with feeding if necessary.

Empowerment & Engagement

• Skill Building: Help individuals with learning disabilities practice and develop independent life skills, such as managing a basic schedule, organizing their room, or simple cooking.
• Cognitive & Social Stimulation: Plan and participate in engaging activities—like puzzles, reading, arts, or conversation—tailored to the cognitive level and interests of seniors and younger clients alike.
• Community Integration: Accompany clients on outings, medical appointments, or community events, serving as a supportive guide and advocate in public settings.

Health, Safety & Administration

• Health Monitoring: Keep a watchful eye on changes in physical health, behavioral patterns, or cognitive function, and promptly report concerns to the care management team.
• Medication Reminders: Ensure clients take the correct medications at the right times, adhering strictly to their individualized care plans.
• Care Documentation: Write clear, accurate, and timely updates in daily care logs regarding the client’s mood, activities, and overall well-being.
• Household Upkeep: Perform light housekeeping duties (such as laundry, dishes, and sanitizing high-touch surfaces) to maintain a safe and dignified living environment.
