We are hiring a Licensed Practical Nurse for a Hospice position providing end-of-life care to patients in their homes in the Leawood, KS. and surrounding areas.
What You’ll Do
- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Conduct ongoing patient assessments focused on comfort, symptom management, and changes in condition.
- Educate and guide patients and caregivers on end-of-life care, symptom relief strategies, and what to expect throughout the hospice journey
- Ensure thorough, compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory

LPN Qualifications
- Active Licensed Practical Nurse (LPN) license in the state you work
- Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
- Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
