Job Description Summary

Responsible for assisting patients and families in the coordination and implementation of their cardiopulmonary rehabilitation plan of care. In collaboration with other members of the team, the Cardiopulmonary Rehabilitation Coordinator ensures high standards of care, pertinent clinical education programs and cost-effective patient management plans.

Job Description

Essential Functions

  • The Cardiopulmonary Rehabilitation Coordinator performs case management core functions.
  • Facilitates the patient/family’s ability to assume responsibility for managing their own health care when appropriate.
  • Works in collaboration with the physicians and other health team members in coordination the rehabilitation plan of care.
  • Ensures collaboration with insurance and managed care companies to help obtain and/or maintain provider status and adequate reimbursement for services.
  • Acts as a resource to members of the health care team in providing patient, family, and staff with information regarding diagnosis, treatments, and therapeutic trends in specialty area.
  • Reviews the requirements for Pulmonary and Cardiac Rehabilitation Program Certification and implements processes to meet the Certification requirements within one year of program implementation.
  • Should be able to perform duties of the Pulmonary Lab Respiratory Therapist.

Education Requirement: Bachelor’s Degree

  • Licensure Requirement: Current Ohio license to practice care
  • Certifications: Registered Respiratory Therapist by the National Board of Respiratory Care required.
  • Additional credential of RPFT and AE-C are strongly encouraged.
