Victorian Post Acute is a 90-bed, rehabilitation-focused skilled nursing facility dedicated to helping residents regain independence, improve function, and enhance their quality of life. We are seeking a compassionate and skilled Occupational Therapist (OTR) to join our exceptional in-house therapy team.

Key Responsibilities

Evaluate residents and develop individualized occupational therapy treatment plans.
Deliver skilled occupational therapy services in accordance with physician orders and established plans of care.
Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with California regulations.
Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to maximize outcomes.
Participate in interdisciplinary care conferences and discharge planning meetings.
Complete timely and accurate documentation, including evaluations, treatment notes, progress reports, recertifications, and discharge summaries.
Educate residents and caregivers on adaptive equipment, safety strategies, home programs, and techniques to maximize independence.
Recommend and coordinate durable medical equipment (DME) as needed.
Help maintain a safe, organized, and efficient therapy environment.

Qualifications

Current California Occupational Therapist (OTR) license in good standing.
Strong communication, organizational, and teamwork skills.
Commitment to providing compassionate, resident-centered care.
