Job Description

Job Summary

The Care Manager at Shriners Hospitals for Children is responsible for coordinating and facilitating comprehensive, patient-centered care for children with complex medical needs, ensuring optimal outcomes and a seamless healthcare experience for patients and families.

Key Responsibilities

  • Perform comprehensive assessments to identify patient and family needs, early in the care process, to ensure smooth progression throughout the continuum of care.
  • Identify logistical needs, including travel, housing, and other non-clinical resources to facilitate access to care and refer to appropriate support staff.
  • Develop individualized, goal driven care plans in collaboration with the interdisciplinary team to optimize patient outcomes.
  • Balance available resources with patient preferences, to facilitate informed decision making.
  • Utilizes knowledge of Medical Necessity, quality of care, and desired outcomes to support appropriate resource management.
  • Support interdisciplinary team education and training.

Required Qualifications

  • RN license in state of practice
  • Basic Life Support (BLS) Required within 6 months of hire

Preferred Qualfications

  • 1-2 Acute Care/Pediatrics/Specialty Case Management Preferred
  • Knowledge of care coordination models and regulatory guidelines (e.g., CMS, URAC)
  • CCM, ACM
  • BSN


Compensation is determined based on years of relevant experience and departmental equity.
