Social Worker LCSW

Social Worker LCSW - REMOTE position. This position is Per Diem and will work 8- hour, Day shifts.

The Transitions Case Manager Licensed Clinical Social Worker (TCM-LCSW) works in a busy environment with a responsibility to communicate with patients, health plans and medical groups. Will work directly with patients and key personnel at each ministry, Care Management, insurance companies, and with our community partners, as needed.

This person exercises considerable initiative and judgment while providing excellent customer service. Follow-up and follow-through are imperative attributes of the TCM-LCSW. This person has a thorough knowledge of services provided throughout the Providence continuum and acts as a resource to the community.

Required Qualifications

• Master's Degree Social Work
• Upon hire: California Clinical Social Worker License or
• Upon hire: California Marriage and Family Therapist License
• Five years Previous experience working in a healthcare role is required.
• Practical experience in customer service.

Preferred Qualifications

• Background in managed care health insurance or care management.
