Work Schedule

100% FTE, day shift working Monday – Friday, 8a – 5p. You will work at the Mile Bluff, Lake Delton and Portage clinics.

We are seeking a Nurse Practitioner or Physician Assistant to

Participate in multidisciplinary HD rounds/ collaborate with the patient's care team for care coordination.
Write and manage dialysis orders
Manage and treat anemia and BMD
Assess and refer for dialysis access placement or intervention
Blood pressure management
Volume status/estimated dry weight assessment
Lab results review/management
Referral for transplant and/or home dialysis program

Education

Minimum –

If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program.
If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program.

Licenses and Certifications

Minimum – State of Wisconsin NP or PA licensure

If Nurse Practitioner*

Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND
Active board certification as a NP in area of clinical practice. Adult; Adult/Gerontology Primary Care, Adult/Gerontology Acute Care.

All Nurse Practitioner and Physician Assistant applicants must have*: Active DEA license or ability to apply for such license prior to or upon hire.
