The MDS Coordinator plays an important clinical and regulatory role within our skilled nursing community. This position is responsible for keeping the MDS process moving—from assessments and documentation review to interdisciplinary coordination, care planning, reimbursement, and survey readiness.
We’re looking for someone who enjoys having ownership of their work, can confidently manage deadlines, and isn’t afraid to ask questions or follow up when something is missing. You’ll work independently while partnering closely with nursing leadership, therapy, social services, and other members of the interdisciplinary team.
This is a deadline-driven and detail-intensive position . There will be days when you are balancing multiple assessments, reviewing clinical documentation, tracking down missing information, responding to changes in resident status, and managing competing deadlines. The right person for this position will find satisfaction in bringing organization to that process and making sure it gets done accurately and on time.

What You Will Do
- Coordinate and complete accurate and timely MDS assessments.
- Oversee the MDS assessment and care-planning process.
- Manage assessment schedules, documentation requirements, and submission deadlines.
- Review clinical documentation for completeness, accuracy, and consistency.
- Identify documentation gaps and follow up with appropriate team members.
- Collaborate with nursing, therapy, social services, and other interdisciplinary team members.
- Develop and update comprehensive, resident-centered care plans.
- Monitor documentation to support regulatory compliance and appropriate reimbursement.
- Provide education and guidance to nursing staff regarding documentation standards and MDS requirements.
- Serve as a clinical resource for MDS, reimbursement, and regulatory processes.
- Support survey preparation and ongoing survey readiness.
- Participate in quality improvement and clinical initiatives.
- Stay current with applicable federal and Wisconsin MDS requirements and regulatory changes.
What You Will Bring
- Current, unrestricted RN license in the state of Wisconsin.
- Minimum of 2 years of MDS experience in skilled nursing or long-term care.
- Strong working knowledge of the MDS assessment and care-planning process.
- Experience managing assessment schedules and regulatory deadlines.
- Experience preparing for and navigating state surveys preferred.
- Strong clinical documentation review skills.
- Exceptional attention to detail and organizational skills.
- Ability to independently manage competing priorities and deadlines.
- Strong communication skills and confidence working across departments.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
