Position Summary
The Discipline Coordinator – Social Work under the direction of the Discipline Head – Social Work will support academic practice, enabling clinicians to work to their full scope of practice and encourages a high-quality workplace that promotes well-being, safety, productivity, and recognition. The Discipline Coordinator exercises exceptional decision-making capabilities, collaboration, as well as facilitation and conflict resolution skills.  The ideal candidate is an exemplary clinician with demonstrated ability to transform and engage the team.  The Discipline Coordinator requires strong verbal and written communication skills, in addition to being able to synthesize information, problem solve and implement strategies to address priorities. 
Duties
- Support the design, implementation and communication of profession standards, competencies, and policies to support profession-specific practice, in keeping with best practice, legislated and professional requirements and UHN’s mission and goals 
- Implement quality indicators and monitoring 
- Support local and corporate initiatives to support profession-specific practice, quality improvement and evidence-based research projects related to practice to improve patient care outcomes. 
- Lead sub-committees and task forces related to emerging social work practice goals, including enhancing the use of Epic, and improving orientation for new hires
- Demonstrate an excellent understanding of the intersecting inequities experienced by patients, and work from a social justice and health equity framework to support social work teams in addressing inequity to improve patient experience
- Plan, facilitate, and evaluate continuing professional development sessions for clinicians, in line with emerging educational needs
- Enable, facilitate and maintain profession specific clinical practice orientation and education pathways to support development from novice to expert 
- Create opportunities to build research and education capacity 
- Make collaborative linkages – with stakeholders in education, research and practice 
- Support UHN Strategic Goals related to social work practice 
- Registration as a Social Worker with the Ontario College of Social Workers and Social Service Workers 
- At least three (3) years’ experience as a social worker required 
- Comprehensive understanding of regulatory standards and legislation related to social work practice 
- Working proficiency with Microsoft Office (e.g. Outlook, Word, PowerPoint, Excel) 
- Strong skills in identifying practice issues and collaboratively problem solving 
- Understands how to create an environment of continuous learning with strong skills in adult education and facilitation 
- Ability to implement quality improvement projects, facilitate knowledge translation and understanding of project management principles 
- Skills in creating an environment that is collaborative, interprofessional and accountable, partnering with patients, clinicians and leaders  
- Previous leadership experience an asset
