Role Overview

Nurse Practitioner Job Description

This is a full-time position, working collaboratively as a member of the medical staff, the APRN will play a critical role in managing the medical needs of skilled nursing facility residents. The APRN will establish and build strong working relationships with administrators and staff throughout their designated building(s) and will act as the primary point of contact for emergencies and urgent care visits. Ultra Health Providers provides our APRNs with the training and tools to do more of what they love – working independently to see patients and treat complex medical conditions.

What Does a Day Look Like for a UHP APRN?

You will provide primary, urgent and preventative care for residents in assigned skilled nursing facility. May also provide care to assisted living facility residents

You will verify admission orders and conduct initial assessment of residents. Conduct physical exams, medication review, and depression screening

You will collaborate with facility Medical Director, attending physicians and nursing staff to deliver the highest level of care to the patient

You will communicate with and counsel family members on treatment options when there are material changes in status

Responsibilities

  • Deliver primary, urgent, and preventive care to residents in skilled nursing and assisted living settings
  • Conduct admission assessments, physical exams, medication reviews, and depression screenings
  • Collaborate with attending physicians, nursing staff, and facility leaders to coordinate care
  • Counsel patients and family members on treatment options and changes in health status
  • Manage clinical documentation accurately and in compliance with facility and state standards

Qualifications

  • Active and unrestricted Nurse Practitioner license in Tennessee
  • Current national board certification (AANP or ANCC)
  • Minimum 1–2 years of experience in primary care, geriatric, or long-term care settings preferred
  • Strong clinical judgment and communication skills
  • Commitment to compassionate, patient-centered care
