Job Title: Physical Therapist (PT) – Full Time

Setting: Skilled Nursing Facility (SNF)

Concept Rehab is looking for a motivated and compassionate Physical Therapist (PT) to join our team in Brighton, MI. This is a full-time opportunity for a PT who enjoys making a meaningful difference in the lives of patients while working alongside a supportive interdisciplinary team.

What You’ll Do

Provide comprehensive physical therapy evaluations and treatments
Develop individualized treatment plans based on patient needs and goals
Help patients improve mobility, strength, balance, and functional independence
Document patient progress accurately and efficiently
Collaborate with nursing, therapy, and other healthcare professionals
Educate patients, families, and caregivers on appropriate therapeutic techniques

What We’re Looking For

Current Physical Therapist (PT) license in the state of Michigan
Graduate of an accredited Physical Therapy program
Strong clinical, communication, and organizational skills
Compassionate and patient-centered approach
