ROLE AND PURPOSE

The Adult Social Care vision in Hull is of ‘a life not a service’. PRINCIPAL ACCOUNTABILITIES

  1. To promote and safeguard the welfare of children, young people and/or vulnerable adults.
  2. Statutory Obligations To provide direct support, guidance, supervision and care which meets the person’s assessed needs, in accordance with their service agreement and agreed care and support plan, on an individual or group basis.
  3. To support with and administer medication in line with the universal medication policy if this is a requirement of the plan following agreed policy and procedures. Maintenance of appropriate records, such as individual medication records and records for checking medication in and out of the building.

To attend to the physical and emotional needs of the people the service supports, whilst always promoting dignity, privacy, independence and personal choice. Tasks include supporting with and undertaking personal care, toileting, washing, dressing, assisting with eating and drinking, PEG feeding, postural management, catheter, abdominal massage, stoma care, and support and direction with following prescribed diets to ensure safe and adequate intake of nutrition and hydration.

To provide encouragement, support and guidance to assist people to completing as much as they can for themselves to encourage development or maintenance of independent skills.

  1. To participate in the identification of needs and reporting of changes and concerns to a line manager, such as changes in moving and handling requirements, changes in health and wellbeing or safety concerns.

Undertake daily record keeping including on digital platforms, and monitoring of health and wellbeing such as completion of mood and behaviour charts, epilepsy monitoring, food intake, dietary, bowel charts and incident reporting forms. following guidance and instruction of senior staff including health colleagues where necessary.

Carry out keyworker/linkworker roles and undertake information gathering for reviews. Working closely with the people we support, their families and other professionals to review the plan on a regular basis ensure consistency and to maximise the effectiveness of the provision and ensure agreed outcomes are achieved.

  1. To support people with behaviours of concern in line with the persons care and support plans, following all relevant legislation, physical intervention training, good practice guidance and least restrictive principles To contribute to the formation, implementation and review of agreed behaviour support plans and risk assessments under the supervision of senior staff and in collaboration with the person, their families, carers and other professionals.
  2. To be aware of and participate in own personal development by attending and contributing to supervision sessions, training courses and other departmental forums. Keep abreast of initiatives and legislation within the care profession by attending in house and/or external training, undertaking e-learning and shadowing.

Work collaboratively with colleagues, managers and other health and social care professionals in line with Hull City Councils Competency Framework to support the city plan in achieving objectives to improve the lives of people we support.

Demonstrate a flexible and adaptable approach, including utilising various communication tools and techniques, using IT equipment and undertaking dynamic risk assessment for unforseen events.

Work cohesively with new colleagues throughout their induction, enabling shadowing to share skills, knowledge and experience to ensure the best outcomes for the people we support and a supportive working environment.

  1. To maintain departmental records, undertake administration functions as required to ensure the accuracy of record keeping, to share the information in a professional manner and to maintain confidentiality as required. To assist people with financial transactions such as dinner monies, to collect monies for payments for activities, transport or meals and to assist senior staff with financial transactions. To lock or unlock buildings/rooms in the absence of senior staff.
  2. To devise and implement interesting, inclusive and meaningful person centred programmes of activity, outings and opportunities for cultural enrichment for people with a range of different disabilities. Promote their mental, physical, social and emotional wellbeing and engagement with opportunities both within the service buildings and within their communities including acting as an escort for personal appointments (where necessary). To take part in the organisation and facilitation of events such as fairs and theme days.
  3. Cook and serve meals, light snacks and drinks taking into account dietary and nutritional needs of service users including assistance to eat and drink where required using various methods such as peg feeding. Completion of appropriate records relating to food handling e.g. temperature recording and cleaning records.
  4. The Health and Safety at Work etc. Act 1974 and associated legislation places responsibilities for health and safety on Hull City Council, as your employer and you as an employee of the council. In addition to the Councils overall duties, the post holder has personal responsibility for their own health & safety and that of other employees; additional and more specific responsibilities are identified in the Council’s Corporate H&S policy.
