About The Role
Job Description Summary
We are seeking a Physical Therapist (PT) to join our Home Health Team in Charlotte County. In this role, there is a real impact in the comfort of patients’ homes. As part of our compassionate, high‑performing care team, you will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to improve mobility, strength, balance, and functional independence. You will collaborate with an interdisciplinary care team, educate patients and families on rehabilitation and injury prevention strategies, maintain accurate clinical documentation, and ensure compliance with all regulatory standards to deliver high-quality patient care and achieve optimal outcomes.
This is a Pro Re Nata (PRN) position with variable time and hours can vary from week to week based on staffing needs, patient volume, call-offs, vacations, or organizational requirements.

How will you make an impact & Requirements
What You'll Do
- Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
- Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management.
- Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long-term health and well-being.
- Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
- Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
- Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.
What We're Looking For
- Licensure: Current and valid Physical Therapist license in the state of Florida.
- Experience: Minimum of 1 year of clinical experience as a Physical Therapist, preferably in a home health or a related setting.
- Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a patient-centered approach to care.
- Professionalism: Strong work ethic, reliability, and a commitment to providing high-quality care.
- Transportation: Reliable transportation to visit patients in their homes.
