Job Description

  • Full-time Hospice Chaplain position
  • Territory: 1-2 counties surrounding the office
  • Performs accurate and comprehensive spiritual assessments within a 5 day window from the start of care date.
  • Ensures that changes in patients’ clinical status are communicated to other healthcare team members and HHA office staff verbally and in writing in a timely manner.
  • Coordinates care planning and delivery with the physician, patient/family, other healthcare team members and HHA staff to facilitate optimal patient outcomes.
  • Acts as liaison between Hospice and the spiritual community.
  • Responds to the mental, spiritual, social and emotional needs of terminally ill patients and their families.
  • Provides nondenominational spiritual counseling per patient and/or family request.
  • Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
  • Communicates appropriately and clearly to physicians, staff, Clinical Nurse Manager and administrative team.
  • Assists family in understanding the grief process.
  • Participates in the bereavement program as requested including facilitating grief support groups, participating in the celebration of life programs, leading funeral services, providing bereavement counseling via phone or in person, sending bereavement mailing, and notifying hospice staff of funeral services.
  • Gives referrals to family for community grief support groups.
  • Allows patient and family to voice their concerns, fears and feelings.
  • Responsive to patient’s desires and condition.
  • Addresses and supports cultural practices, as long as such practices do not harm others.
  • Performs accurate and comprehensive bereavement assessments. Assesses the social and emotional factors related to family members/survivors/primary caregiver coping with a loss.
  • Develops and implements the care plan for bereaved according to grief risk and contact frequency. Provides, modifies or discontinues bereavement interventions based upon an ongoing assessment/reassessment of the bereaved needs.
  • Maintains up to date knowledge of Medicare Hospice bereavement guidelines and ensures compliance.
  • Provides bereavement calls and support to beavered according to the level of need.
  • Conducts one-on-one counseling and grief education sessions for bereaved as determined by need. Refers bereaved to appropriate mental health/community support programs as appropriate.
  • Sends mailings to the bereaved including newsletters, condolence cards, and letters.

Requirements

  • Master's Degree in Ministry, Counseling or required
  • Current driver's license
  • Must have reliable transportation and insurance
  • One year Hospice experience is preferred, but not required
