About the Job
The Licensed Practical Nurse (LPN) Clinical Quality Navigator supports agency quality outcomes through proactive telephonic outreach, patient triage, care coordination, and follow-up across the episode of care. This role helps identify clinical concerns early, promote patient satisfaction, support timely planned and unplanned clinical visits, and reduce avoidable readmissions while ensuring smooth coordination of services. The LPN Clinical Quality Navigator also supports clinical managers and scheduling staff by helping manage follow-up needs, priority workflows, and promote timely coordination of care across the agency.
What You’ll Do

- Conduct timely post-discharge telephonic follow-up with patients to assess clinical status, reinforce discharge instructions, and identify potential risks or unmet needs.
- Promote patient satisfaction by addressing questions, concerns, and barriers to care in a professional, compassionate, and responsive manner.
- Assist in identifying trends or gaps impacting patient satisfaction and quality outcomes and communicate findings to leadership.
- Monitor and document all patient interactions accurately in the electronic medical record in accordance with agency policy and regulatory standards.
- Serve as a resource to patients and caregivers, ensuring continuity and clarity of care following discharge from home health services.
- Support agency initiatives related to Value-Based Purchasing (VBP) and QAPI, including patient experience, care coordination, and outcome improvement.
- Assist with quality audits as assigned
- Assess patient-reported symptoms and conditions via telephone and apply clinical judgment to triage needs appropriately.
- Coordinate with clinical managers to escalate concerns, initiate interventions, or adjust care plans based on patient status.
- Collaborate with scheduling staff to prioritize and facilitate timely planned and PRN visits based on patient acuity and needs.
- Support prevention of avoidable hospital readmissions through early identification of clinical
deterioration and promptly escalating concerns through established clinical and/or disease specific protocols and notification to the provider.
- Reinforce patient and caregiver education related to disease management, medications, and when to seek additional care
Who You Are
- Education: Graduate of an accredited school of Licensed Practical Nursing required.
- Certification/Licensure : LPN NH licensure required. Current BLS certification or must obtain within 90 days of date of hire required.
- Experience : Two (2) plus years nursing experience required. Home Health experience preferred.
- Abilities : Ability to perform nursing triage
- Schedule: Work schedule may include eight (8) hours per day. Staff are assigned to the services area of VNA to meet client/patient needs.
- Other: Valid Driver’s License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities. Proof of auto mobile insurance required. Motor vehicle background report will be conducted.

Work Shift
8am-4:30pm. (No weekend requirements)
