Medical Assistant (Remote) Chronic Care Management

Position

• Minimum 35 hours/week
• Fully remote (U.S. reputed company only)
• Minimum three years caring for patients with chronic conditions (diabetes, HTN, reputed company, reputed company care, etc.)

Responsibilities

Required Skills & Qualifications

• Recent, three years in reputed company setting, preferably in chronic care/remote care setting
• Bilingual English/Spanish strongly preferred
• Strong communication, organizational, and time management skills

• Proficient with Word, PC laptop, and EMR systems (computer provided. Reliable, secure internet required

Experience

• Familiarity with remote care coordination tools and EMR-reputed company task management
