MEDICAL DIRECTOR JOB SUMMARY

The Hospice Physician is a licensed medical doctor responsible for providing comprehensive hospice and palliative medical care to patients with terminal illnesses. The physician evaluates and diagnoses medical conditions, develops and oversees plans of care, prescribes treatments and medications, and collaborates with patients, families, and the interdisciplinary hospice team to ensure quality end-of-life care. The Hospice Physician performs medical assessments, face-to-face encounters, and hospice eligibility evaluations in accordance with Medicare Conditions of Participation, state and federal regulations, and organizational policies. The physician serves as a clinical leader and resource to the interdisciplinary team while supporting patients and families throughout the hospice journey.

Revised Duties & Responsibilities

Patient Care

Provides comprehensive medical management of hospice and palliative care patients in accordance with applicable state and federal regulations.
Conducts medical evaluations, reviews patient histories, performs physical examinations, and documents findings in the medical record.
Diagnoses and manages terminal illnesses and related symptom burdens through evidence-based medical treatment plans.
Prescribes medications, treatments, and interventions necessary for pain and symptom management.
Conducts hospice face-to-face encounters prior to the third benefit period and subsequent recertification periods as required by Medicare regulations.
Evaluates and certifies or recertifies patient eligibility for hospice services based on clinical findings and regulatory requirements.
Reviews clinical information and consults with members of the interdisciplinary team regarding patient care needs and treatment planning.
Provides medical oversight and guidance regarding symptom management, disease progression, and end-of-life care planning.
Collaborates with attending physicians, specialists, and community healthcare providers to ensure continuity of care.
Establishes and reviews clinical protocols and participates in the development of standards of care.
Assists in identifying opportunities for quality improvement and clinical excellence.
Participates in after-hours or on-call medical coverage as assigned.

Communication

Collaborates with the hospice interdisciplinary group to establish, review, and revise individualized plans of care.
Communicates regularly with attending physicians, specialists, advanced practice providers, and other healthcare professionals regarding patient status and treatment recommendations.
Educates patients, families, caregivers, and staff regarding disease processes, prognosis, symptom management, and goals of care.
Participates in hospice interdisciplinary team meetings and case conferences.
Completes and maintains accurate, timely, and compliant medical documentation.
Provides medical consultation and clinical leadership to members of the hospice team.
Supports community outreach and professional relationships that promote hospice services and organizational growth.

Additional Duties

Maintains current knowledge of hospice regulations, Medicare Conditions of Participation, and applicable state and federal requirements.
Ensures compliance with HIPAA and organizational policies.
Participates in quality assurance, performance improvement, and utilization review activities.
Serves as a clinical resource and mentor to interdisciplinary team members.
Maintains professional licensure, certifications, and continuing medical education requirements.
Demonstrates expertise in the management of end-of-life care, symptom control, and goals-of-care discussions.

MOBILE PCP JOB SUMMARY

The Mobile Primary Care Physician is a licensed physician responsible for providing comprehensive primary care services to patients in their homes, assisted living facilities, independent living communities, group homes, and other community-based settings. The physician delivers preventive, acute, chronic, and transitional care services while promoting patient independence, reducing avoidable hospitalizations, and improving overall health outcomes.

The Mobile Primary Care Physician performs medical assessments, diagnoses and treats illnesses, prescribes medications and treatments, develops individualized plans of care, and coordinates services with patients, caregivers, specialists, community resources, and interdisciplinary healthcare teams. The physician serves as a clinical leader and advocate, fostering high-quality patient-centered care through evidence-based practice and proactive disease management.

DUTIES & RESPONSIBILITIES

Patient Care

Provides comprehensive primary care services in patients' homes and community-based settings.
Obtains and documents complete medical histories and performs physical examinations.
Evaluates, diagnoses, and treats acute and chronic medical conditions.
Develops, implements, and monitors individualized treatment plans.
Prescribes medications, treatments, and therapeutic interventions as appropriate.
Orders and interprets laboratory tests, imaging studies, and diagnostic procedures.
Conducts annual wellness visits, routine preventive care screenings, and health maintenance evaluations.
Performs transitional care management following hospitalization, emergency department visits, or skilled nursing stays.
Provides chronic disease management for conditions such as hypertension, diabetes, congestive heart failure, COPD, dementia, and other complex medical conditions.
Identifies patients who may benefit from palliative care, specialty referrals, or additional support services.
Coordinates care with specialists, hospitals, home health agencies, hospice agencies, rehabilitation providers, and community services.
Participates in advance care planning discussions and assists patients with healthcare decision-making.
Maintains timely and accurate clinical documentation in the electronic medical record.
Participates in after-hours coverage and call responsibilities as assigned.

Communication & Care Coordination

Establishes therapeutic relationships with patients, families, caregivers, and healthcare partners.
Collaborates with interdisciplinary team members to coordinate and optimize patient care.
Communicates effectively with consulting physicians, specialists, hospitals, and post-acute care providers.
Educates patients and caregivers regarding diagnoses, treatment plans, medications, preventive health measures, and disease management strategies.
Participates in clinical team meetings, case reviews, and care conferences.
Documents and communicates changes in patient condition promptly and appropriately.
Facilitates coordination of community resources and supportive services.
Promotes continuity of care across healthcare settings.

JOB REQUIREMENTS

Required Education

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school.

Family Medicine
Internal Medicine
Geriatric Medicine

Other applicable primary care specialty

Licensure & Certification

Current unrestricted medical license in the state of employment.

Current CPR/BLS certification.
Board Certification or Board Eligibility in Family Medicine, Internal Medicine, Geriatrics, or related specialty preferred.

Experience

Minimum one (1) year of clinical practice experience preferred.
Experience in home-based primary care, geriatrics, value-based care, post-acute care, or population health management preferred.
Experience caring for medically complex and underserved populations preferred.

Knowledge & Skills

Strong knowledge of evidence-based primary care medicine.
Thorough understanding of chronic disease management and preventive care principles.
