We are hiring a Spiritual Care Counselor for a Hospice position providing care to patients in their homes in the Troy area.
What You’ll Do
- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
- Conduct spiritual assessments to identify patient and family needs, helping develop care plans that support emotional, spiritual, and end-of-life goals
- Provide spiritual counseling, emotional support, and guidance to patients and families of diverse backgrounds and beliefs, helping them navigate the end-of-life journey with comfort, dignity, and compassion
- Address complex emotional and spiritual needs, advocate for patient wishes, and maintain accurate, timely, and compliant EMR documentation
- Work independently but never alone
- Travel to patient homes within assigned territory

Spiritual Care Counselor Qualifications
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred
- Experience working with death and dying, individuals/family/caregiver
- Hospice experience preferred
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
