We are hiring a Spiritual Care Counselor for a Hospice position providing care to patients in their homes in the Troy area.

What You’ll Do

Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
Conduct spiritual assessments to identify patient and family needs, helping develop care plans that support emotional, spiritual, and end-of-life goals
Provide spiritual counseling, emotional support, and guidance to patients and families of diverse backgrounds and beliefs, helping them navigate the end-of-life journey with comfort, dignity, and compassion
Address complex emotional and spiritual needs, advocate for patient wishes, and maintain accurate, timely, and compliant EMR documentation
Work independently but never alone
Travel to patient homes within assigned territory

Spiritual Care Counselor Qualifications

Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred
Experience working with death and dying, individuals/family/caregiver
Hospice experience preferred
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
