Conduent is seeking a compassionate and detail-oriented Information & Referral Specialist to help connect individuals, families, seniors, and people with disabilities to critical programs, services, and community resources. In this role, you will serve as a trusted resource by conducting assessments, identifying service needs, and providing referrals that improve access to care and support services.

What You’ll Do

  • Conduct Level of Care Eligibility Tool (LOCET) assessments and other required evaluations.
  • Manage high-volume inbound and outbound calls with professionalism, empathy, and accuracy.
  • Connect individuals to appropriate community resources, social services, Medicaid programs, and support services.
  • Assess caller needs and identify available solutions based on program eligibility and service availability.
  • Research and provide referrals to non-Office of Aging and Adult Services (OAAS) community programs.
  • Document interactions accurately while maintaining confidentiality and compliance requirements.
  • Deliver exceptional customer service while supporting aging adults, individuals with disabilities, caregivers, and families.

Experience We’re Looking For

  • Bachelor’s degree in Human Services, Social Work, Psychology, Sociology, Counseling, or a related field.
  • Minimum of 1 year of experience in social services, human services, case management, customer support, healthcare support, or a related field.
  • Experience working with elderly adults, individuals with disabilities, caregivers, or underserved populations.
  • Knowledge of OAAS, Medicaid, BHSF, DHH, OCDD, and Home and Community-Based Waiver Programs.
  • Understanding of medical and developmental disability terminology.
  • Strong communication, problem-solving, research, and resource navigation skills.
  • Ability to maintain strict confidentiality, work independently, and remain composed under pressure.
  • Proficiency with Microsoft Office Suite including Word, Excel, Outlook, and Teams.

Work Schedule & Location

  • Must attend onsite training in Baton Rouge, Louisiana for the first six weeks.
  • Training schedule: Monday through Friday, 8:00 AM to 5:00 PM.
  • Hybrid work arrangement available following successful completion of training, Monday through Friday, 8:00 AM to 5:00 PM.

Preferred Qualifications

  • Additional experience supporting seniors, individuals with disabilities, or Medicaid recipients.
  • Experience conducting assessments, eligibility reviews, or case management activities.
  • Advanced knowledge of OAAS programs, Medicaid services, long-term care programs, and community-based resources.
  • Experience in call center, healthcare, government services, social services, or public assistance environments.

For this position, actual salaries will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. In addition to base pay, this position, based on business need, may be eligible for a bonus or incentive. In addition, Conduent provides a variety of benefits to employees including health insurance coverage, voluntary dental and vision programs, life and disability insurance, a retirement savings plan, paid holidays, and paid time off (PTO) or vacation and/or sick time. The estimated hourly rate for this role is $17.50.
