About The Role

ESSENTIAL FUNCTIONS

  • Provide Recovery-Focused Case Management: Support individuals with mental health and/or
  • substance use challenges by developing personalized recovery plans, identifying goals, and
  • coordinating services to promote long-term stability and independent living.
  • Facilitate Service Linkage: Connect individuals to treatment programs, housing, benefits
  • assistance, employment resources, healthcare providers, transportation services, and other
  • needed community supports.
  • Conduct Comprehensive Assessments: Gather and evaluate information related to individuals’
  • mental health, substance use, social, vocational, and housing needs to inform service planning
  • and interventions.
  • Monitor Progress Toward Goals: Maintain regular contact with clients, evaluate progress, and
  • revise recovery plans as needed. Provide advocacy and problem-solving support in overcoming
  • service barriers.
  • Promote Client Empowerment: Encourage self-determination, resilience, and skill-building in

alignment with trauma-informed and recovery-oriented care principles.

  • Maintain Timely Documentation: Document all services, contacts, referrals, and progress notes in

compliance with agency policies, Medicaid billing requirements, and confidentiality laws.

  • Collaborate with Team and Community Partners: Participate in team meetings, interdisciplinary
  • case reviews, supervision, and trainings. Communicate effectively with community providers and
  • stakeholders to ensure holistic and coordinated care.
  • Respond to Crises: Provide supportive interventions during emotional or environmental crises and

link clients to appropriate emergency services when needed.

  • Work Expectations

Maintain a flexible schedule, including occasional evenings or weekends as needed. Work in varied environments including community locations, homes, and service agencies. Report any changes that may affect licensure, credentialing, or ability to perform job functions.

WORK EXPERIENCE REQUIREMENTS

  • Relevant work experience in mental health, substance use, or case management settings strongly
  • desired. Must have valid driver’s license and reliable transportation. Ability to develop positive
  • rapport, work collaboratively, and demonstrate empathy and cultural humility. Knowledge of
  • trauma-informed care, motivational interviewing, and local service systems required.
  • Uphold all requirements of employment including transportation, communication, and attendance
  • standards.

EDUCATION REQUIREMENTS

High School Diploma or equivalent required.
