About The Role

LCSW Care Manager Psych at Providence Little Company of Mary Medical Center San Pedro in San Pedro, CA. This position is Full Time and will work 8-hour, Day shifts.

Care Management within the Department of Psychiatry includes a collaborative practice with patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community. Social workers facilitate communication and care coordination along a continuum through effective transitional care management. Recognizing the patient's right to self-determination, the significance of the social determinants of health and the complexities of care (barriers to coping, abuse, mental health, substance use, lack of resources, psychosocial issues, etc.).

Required Qualifications

  • Master's Degree in Social Work.
  • Coursework/Training: All LCSWs must follow the guideline by Los Angeles County Department of Mental Health for LPS designation and keep their designation in current status.
  • Upon hire: California Clinical Social Worker License
  • Designated as Behavioral Health Clinician to perform 5150 evaluations within 18 months of hire
  • CPI certification (nonviolent crisis intervention training) within 1 year of hire
  • 2 years Healthcare related setting with psychiatric mental health patient population in area of service, preferably in an inpatient setting.
  • Computer experience.

Preferred Qualifications

  • Discharge planning experience.
