About The Role

Under the supervision of the Nurse Manager, the Licensed Vocational Nurse – Juvenile Justice Center provides direct nursing care to adolescents and young adults receiving healthcare services at the Juvenile Justice Center through UCSF Benioff Children’s Hospital.

The LVN supports the delivery of safe, developmentally appropriate, trauma-informed, and patient-centered care within a secure juvenile justice setting.

Responsibilities include performing patient intake and obtaining vital signs, administering medications and immunizations, performing ordered treatments and laboratory procedures, assisting with health screenings and clinical assessments, and supporting providers during examinations and procedures within the LVN scope of practice. The LVN coordinates care among patients, healthcare providers, interdisciplinary team members, and Juvenile Justice Center staff; recognizes and communicates changes in patient condition; maintains accurate and timely documentation in Epic; and supports continuity of care and follow-up services.

The LVN maintains patient privacy and confidentiality and follows established infection prevention, medication safety, security, and clinical protocols specific to the Juvenile Justice Center environment. The position promotes equitable, culturally responsive, trauma-informed care for a diverse adolescent population while adhering to UCSF policies, professional standards, and Living PRIDE values.

  • Identifies the needs and responses of patients to medical regimen and treatment through observation of symptoms, emotional state, and cultural-socioeconomic backgrounds.
  • Recognizes and reports specific physical and emotional conditions/needs/concerns of the patient to appropriate members of the health care team. Alerts a provider of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath.

  • Prepares for and instructs patients about specific diagnostic and therapeutic procedures and treatments.
  • Assists as needed with diagnostic and therapeutic procedures and treatments.
  • Obtains specimens and performs Point of Care Testing as directed. Performs QA/QC on CLIA waived tests and reports any tests out of QA range to supervisor.
  • Uses appliances and equipment correctly and maintains safe practice.
  • Evaluates and documents patients’ responses to nursing care, including pre/post procedures and treatments. Uses knowledge of cognitive, physical, emotional and chronological maturation process in evaluating care for all ages of patients.
  • Responds appropriately in emergencies. Understands the procedures for medical emergencies, code blue and other emergency/disaster situations. Participates in safety drills.
  • Complies with all Prepare Clinic and Medical Center policies and procedures for infection control, safety, administrative and clinical practice. Complies with activities mandated by JCAHO/Title 22/NCQA.
  • Completes annual competency testing as specified by the individual practice.

  • Organizes activities based on the needs of patients while maintaining the effective flow of patients through the practice. Adjusts clinical support activities to reflect changes in priorities during practice sessions.
  • Maintains open communication and facilitates continuity of care for patients/and or family with members of the health care team through phone contact, verbal interaction and all computer systems information (i.e., labs, appointments) to contribute to patient care.

  • Participates in identifying and documenting the educational needs of patients.
  • Provides patients with the necessary information to make informed decisions utilizing available resources and making referrals appropriately. Informs patients/families about services available at UCSFMedical Center and in the community. Utilizes and maintains updated educational resources.

Required Qualifications

  • One year of correctional nursing experience
  • Phlebotomy certification
  • EKG experience
  • Knowledge and abilities essential to the successful performance of the duties assigned to the position, including demonstrated excellent communication skills, problem-solving ability, and high comfort level with learning new technology/systems
  • Basic computer/keyboarding skills
  • Prior experience with EPIC
  • Demonstrated ability to work in a fast-paced environment

Required Licenses/Certifications

  • Licensed as a Vocational Nurse in the State of California
  • AHA CPR certification (BLS)
