About The Role
Utilizes the nursing process for assessment, planning, implementation, and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with more complex needs with assistance and supervision. Assists with the management of patient care administered by other members of the nursing team.

YES Diuretic Clinic - IV initiation and maintenance, IV push and oral medication administration, phlebotomy. Fall risk prevention and patient response to medication therapies. Appropriate department required documentation in the electronic health record including charge capture for relevant services. Other patient care needed to ensure safe outcomes

YES Amvuttra Clinic- enrollment with drug therapy company, ensures insurance approvals, often assisting with Prior Authorization process, medication injection. Appropriate department required documentation in the electronic health record including charge capture for relevant services. . Other patient care needed to ensure safe outcomes

Yes Heart Failure Remote Monitoring Program – review physiological data for evaluation of HF status, implementing changes in the plan of care based on standard practices of the department. Other patient care needed to ensure safe outcomes

REQUIRED QUALIFICATIONS
- 1 year of nursing recent experience caring for cardiac patients in an acute care or ambulatory setting with working knowledge of diuretics and electrolytes in the Heart Failure population.
- This position requires flexibility to orient and work at all UCSF Medical Center locations.
PREFERRED QUALIFICATION
- Proficiency with IV initiation and IV push medications.
- Experience in caring for Advanced Heart Failure patients
- Proficiency with Epic electronic health record
- Ability to multitask and easily float between clinic responsibilities
- BSN
REQUIRED LICENSES/CERTIFICATIONS
- Current licensure as a Registered Nurse in the State of California.
- Current healthcare provider CPR certification
