Key Responsibilities

PT Responsibilities

  • Perform comprehensive physical therapy evaluations in patient homes.
  • Develop individualized treatment plans focused on functional mobility, safety, and independence.
  • Provide therapeutic interventions including gait training, strengthening, balance work, and home‑exercise program development.
  • Conduct home‑safety assessments and recommend appropriate DME or environmental modifications.
  • Manage a field‑based caseload with strong organizational efficiency.
  • Collaborate with interdisciplinary team members to ensure coordinated, patient‑centered care.
  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Schedule

Full‑Time Schedule

  • Monday–Friday
  • Daytime schedule
  • Balanced caseload with strong team support

Qualifications

PT Requirements

  • Current Idaho Physical Therapist license (or ability to obtain).
  • One year of PT experience preferred; home‑health experience a plus.
  • Strong communication, assessment, and clinical reasoning skills.
  • Ability to work independently in a field‑based environment.
  • Reliable transportation and valid driver’s license.

Compensation & Benefits

Compensation Details

  • Competitive salary: $105,000–$135,000
  • $10,000 sign‑on bonus
