About The Role
Liquid Personnel is recruiting for an experienced Locum Occupational Therapist to join its client in Wiltshire Council’s Reablement Team, supporting individuals following hospital discharge to regain independence and achieve positive outcomes within the community.
This is a fantastic opportunity for an experienced OT with reablement, community, hospital discharge or Adult Social Care experience, working across home, hospital and community settings.
What will your responsibilities be?
Complete Occupational Therapy and functional assessments for individuals following hospital discharge
Develop tailored reablement plans focused on restoring independence and reducing ongoing reliance on care and support
Work across home, hospital and community settings to assess functional needs and identify practical solutions
Provide specialist advice around equipment, safe practice, mobility, transfers and independence
Work collaboratively with Social Workers, health professionals, care providers and the wider multidisciplinary team
Support decision-making around care, risk, rehabilitation and ongoing support needs
Monitor progress and review interventions to ensure reablement goals remain appropriate and outcomes-focused
Maintain accurate and timely clinical records and manage a varied caseload effectively
Promote person-centred, strengths-based practice, enabling individuals to maximise their independence following hospital discharge

Qualifications & Experience
Qualified Occupational Therapist
Current HCPC registration
Minimum 2 years' post-qualification experience
Experience in reablement, community OT, hospital discharge or Adult Social Care is desirable
Strong assessment, clinical reasoning and risk management skills
Full UK driving licence and ability to travel to community visits
