What You'll Do

The Lead Case Manager provides direct clinical case management services while serving as a clinical resource and workflow lead within the walk-in, Observation, and CSU units. This position coordinates patient care, facilitates interdisciplinary communication, supports treatment planning and discharge coordination, and ensures timely and compliant clinical documentation. The Lead Case Manager promotes patient-centered, recovery-oriented care while supporting patient flow, continuity of care, and consistent application of established clinical and operational standards.

The Lead Case Manager models clinical excellence, collaboration, and professionalism while serving as a peer resource to support efficient unit operations and high-quality patient outcomes.

Clinical Care Coordination & Treatment Planning

  • Manage daily patient cases across the walk-in, Observation, and CSU units by facilitating admission assessments and developing individualized, person-centered stabilization or treatment plans designed for brief intervention.
  • Complete, review, and obtain signatures for treatment plans with patients and involved parties, ensuring understanding and engagement in the treatment process.
  • Gather collateral information from patients, family members, and other involved parties to support comprehensive assessment, treatment planning, and discharge recommendations.
  • Coordinate patient care with the interdisciplinary treatment team to ensure services are integrated, patient needs are met, and safety is maintained throughout the episode of care.
  • Identify service gaps and facilitate appropriate referrals and discharge planning to ensure continuity of care and successful transition to community-based services.
  • Monitor Observation Unit census and available community placement options to support timely patient flow and disposition planning.

Documentation & Compliance

  • Maintain accurate, timely, and complete clinical documentation within the medical record, including assessments, stabilization and treatment plans, brief interventions, care coordination activities, and discharge planning.
  • Obtain required patient signatures and ensure all treatment-related documentation complies with organizational, regulatory, and accreditation standards.

Communication & Team Collaboration

  • Provide comprehensive daily shift reports to the Director, Clinical Services and oncoming case managers to ensure continuity of patient care and operational awareness.
  • Collaborate effectively with physicians, nursing staff, behavioral health professionals, and community partners to coordinate patient care and achieve positive treatment outcomes.
  • Participate in individual and group supervision, interdisciplinary case reviews, and team meetings to support quality care and professional development.
  • Provide shift-based workflow guidance by communicating patient care needs, prioritizing case management tasks, and supporting timely coordination with the interdisciplinary team.
  • Serve as a peer clinical resource to case management staff by sharing information, reinforcing established processes, and supporting consistent application of organizational standards and care coordination practices.
  • Share shift observations, workflow barriers, and patient care escalation needs with the Director, Clinical Services and/or appropriate leadership to support coaching, operational awareness, and timely issue resolution.
  • Escalate patient care, workflow, or documentation concerns to the Director, Clinical Services and/or appropriate leadership to support timely resolution and continuity of care.

Professional Development

  • Participate in ongoing education, in-service trainings, and competency-based learning activities to maintain clinical knowledge and professional effectiveness.
  • Demonstrate competency through post-testing, skill observations, and performance evaluations conducted by leadership.

Additional Duties

  • Performs additional duties and special projects as assigned in support of departmental operations and patient care.

What You'll Bring

​ Master's degree in Counseling, Social Work, Marriage and Family Therapy, or a related behavioral health field, and current Pennsylvania licensure in good standing as one of the following:

  • Licensed Social Worker (LSW)
  • Licensed Associate Counselor (LAC)
  • Experience providing peer guidance, shift-based lead support, or workflow coordination for clinical or case management staff
  • Working knowledge of federal and state behavioral health or hospital operations and regulatory requirements
  • Must be able to perform and certify Safe Clinch and Therapeutic Options patient restraint techniques
  • The Company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the Company’s exemption process prior to their start date as a condition of employment
  • Complete all credentialing requirements within the required time period, including but not limited to obtaining a National Provider Identifier (NPI), Medallion enrollment, Medicare enrollment, and any other necessary credentials as required for the role
  • Must pass the following background checks
  • Child Abuse Clearance
  • FBI Criminal Background Check
  • Pennsylvania State Police Criminal Record Check
  • Medical screenings (Physical, TB, Drug test)

It would be great if you had

  • BLS (Basic Life Support) must be obtained within seven days of hire
  • Knowledge or the familiarity with the 302 involuntary process
  • Experience in multiservice hospital environment with matrix reporting structures
