About The Role
Lakeview is looking to hire a Home Health Aide to join our Homecare/Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary
Assists professional personnel to provide patient care according to department and hospital standards. Completes other supportive tasks necessary to the functioning of the department under the supervision of a Case Manager.
Essential Duties
- Demonstrates competence in performance of tasks.
- Vital signs
- Weigh patients
- Transfer patient safely
- Specimen collection
- Aseptic Technique
- Clean dept specific equipment and supplies
- Documents accurate vital signs and I & O
- Assists patients with ADLs and performs other tasks as assigned
- Foley catheter care
- Simple dressings
- Prepare meals, feed
- Observe safety issues in the home
- Apply teds, ace wraps, slings etc.
- Bathing with or without assistance
- Foot soaks
- Shampoo hair
- File fingernails and toenails
- Combing/brushing hair, makeup and shaving
- Perform personal hygiene tasks clients are unable to perform without assistance
- Perform total bath, requiring turning and repositioning
- Perform perineal care
- Perform skin care measures, e.g. application of barrier cream or lotion
- Provide for oral /teeth care.
- Perform or assist the client in performing activities related to mobility-passive range of motion, turning and repositioning
- Do routine ostomy cares e.g. change or empty the bag only. No wafer cares
- Perform or assist the patient in performing the normal activities of daily living such as eating and dressing
- Assist the patient in taking self-administered oral medications
- Perform light housekeeping duties to maintain the patient’s level of function: prepare light meals, change bed linens, clean and straighten the patient’s immediate personal environment.
- Report significant observations and changes in the patient’s condition to the CM within a timely manner.
- Oxygen per patient request.
- Communicate pertinent data clearly, concisely and in a timely manner to the Case Manager.
- Maintain department order and equipment management as needed.
4.Performs other tasks as required or assigned.
- Point of care documentation into EHR.
6.Driving to client’s residence.
Work Schedule
0.6 FTE (scheduled 3 days per week). Working hours are 8-4:30 pm. No weekends or holidays.
Required Qualifications
- Education
- High school diploma or equivalent
- License and Certification:
- Must have a valid driver’s license and meet all criteria required under Lakeview’s Automobile Liability Guidelines
- Must have MN or WI CNA certificate upon start date
- Must provide proof of MN and WI CNA certificates within 4 months of start, if both MN and WI CNA certificates are not present on start date
- Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)
- Experience / Skills
- Must be 18 or older and have at least 1 year of driving experience
