About The Role

Lakeview is looking to hire a Registered Nurse to join our Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.

Position Summary

Provides skilled nursing assessments of client/caregivers in the Homecare setting which includes physical, psychosocial and environmental assessment and care. Develops a care plan in consultation with the client /family unit, the interdisciplinary team, and the primary physician. Coordinates and plans with the professionals and caregivers involved, to meet the goals of the client’s plan of care. Supervision of LPNs and HHAs.

Work Schedule: On Call - Day shift, 8a-4:30p as needed. Minimum of 32hr/month availability.

Required Qualifications

Education, Experience or Equivalent Combination:
Associate’s nursing degree from an accredited school
Licensure/ Registration/ Certification:
Must have MN or WI RN license upon start date.
Must provide proof of MN and WI RN license within 4 months of start, if both MN and WI RN licenses are not present on start date.
Must have a dependable vehicle, valid driver’s license, auto insurance and meet all criteria required under Lakeview’s Automobile Liability Guidelines
Knowledge, Skills, and Abilities:
Knowledge of reimbursement procedures as well as knowledge of community resources
Accept responsibility for maintaining current knowledge level of nursing trends in the Hospice field.
Able to function independently with minimal supervision in home setting
Strong assessment skills
Ability to read, write and speak the English language
Ability to carry out directions
Computer experience
Comfortable in end-of-life care
Flexible with changing schedule
Willing to participate in On-Call rotating schedule

Preferred Qualifications

Education, Experience or Equivalent Combination:
Bachelor’s in nursing from an accredited school
Master’s in nursing from an accredited school
Licensure/ Registration/ Certification:
Proof of MN and WI RN license within 4 months of start, if both MN and WI RN licenses are not present on start date.
Knowledge, Skills, and Abilities:
One year experience of Homecare, Hospice, or Long-Term Care
