About The Role

Temporary/Fixed term: 12 months

We are looking for a Transforming adult social care service delivery lead. This role will play a key part in supporting the design, delivery and ongoing improvement of new Adult Social Care service models. The post holder will work collaboratively across teams, ensuring new ways of working are effectively embedded and delivered, driving better outcomes for the people we support.

As Service Lead, you will play a pivotal role in shaping and improving Adult Social Care. You will foster strong partnership across service and drive improvements in quality, accessibility, and overall experience.

The post holder will provide expert advice and guidance to the Front Door Service. They will oversee key areas of improvements including enhancing Technology Enabled Care, strengthening staff training, building links with community and voluntary organisations, improving CRM systems and workflows and developing the digital front door to support self-referrals. These priorities will continue to evolve over time.

The successful candidate will need a professional qualification in health and social care. This could be a Level 5 qualification in Leadership and Management, the relevant qualification in Social Work or Occupational Therapy or the ability to demonstrate relevant experience.

You will need to evidence experience of working at a Senior level within a complex organisation, with strong leadership qualities, project management experience a positive approach to collaboration and an ability to deliver against deadlines.

  • Coordinate delivery of Front Door transformation workstreams, ensuring alignment and timely progress
  • Lead implementation of service improvements, including digital access, CRM enhancements, and Technology Enabled Care (TEC)
  • Strengthen partnership working with community, health, and voluntary sector partners
  • Use performance data and feedback to inform decision-making and service redesign
  • Identify and manage risks, issues, and dependencies across the programme
  • Provide clear reporting and briefings to senior leadership and governance forums
  • Support embedding of consistent, person-centred and strengths-based practice
  • Relevant degree or equivalent experience in Adult Social Care, health, or a related field
  • Professional qualification (e.g. Social Work, Occupational Therapy) with current registration
  • Evidence of leadership and programme/change management capability
  • Significant experience at a senior level within a complex organisation
  • Strong understanding of Adult Social Care legislation, safeguarding, and front door services
  • Experience of leading transformation, service redesign, and partnership working
