Community Support Worker

REPORTS TO: Clinical Manager

PURPOSE

  • Under the direction of a clinical manager, the Community Support Worker provides support
  • to consumers and their families or caregivers with mental health and substance abuse
  • related issues. The Community Support Worker will facilitate individual mental health
  • intervention and activities that are strength‐based and focused on promoting recovery,
  • symptom reduction, while increasing coping skills and help to achieve the highest level of
  • functioning in the community.

EDUCATION& EXPERIENCE

  • Bachelor’s degree or at least 2 years of related work experience

GENERAL RESPONSIBILITIES

  • Ensure all necessary licenses and/or certifications remain current
  • Provide medically necessary services to the consumer in accordance with the
  • treatment plan that is designed to meet the mental health, educational, vocational,
  • residential, financial, social and other needs of the consumer.
  • Follow Mental Health Rehabilitation Services (MHRS) guidelines
  • Ensure that treatment plan authorizations and reauthorizations, CALOCUS/LOCUS
  • and updated diagnostic assessments and all other assessments are current and DO
  • NOT expire
  • Serve as an intricate member of the SLS team to ensure continuity of care and

supports the services 24 hours a day and seven days per week, as necessary

  • Provides structured face‐to‐face scheduled sessions to consumers at least once per

month, or as needed based on the treatment plans

  • Provide services in the home, school, office and community setting
  • Ensure that the majority of mental health services are primarily rendered in the

community and limited services remain clinic-based.

  • Complete documentation within 24 hours of service or report to supervisor via phone
  • Participate in mandatory training
  • Assist with the facilitation of staff meetings and trainings
  • Collaborate with and provide ongoing updates to the clinical team and psychiatrist,

as needed

  • Identify, establish, and continually collaborate with community agencies, providers,

and natural supports and advise team to make referrals as needed

  • Listen/respond to and interact with clients, co‐workers, management, and other

professionals in a non‐judgmental manner

  • Communicate objectively, thoroughly, accurately and concisely in a manner

consistent with the cognitive level of the persons involved

  • Demonstrate an understanding and acceptance of the organizational mission and

shared purpose to assist individuals by providing quality services

  • Participate in efforts to enhance service provision to consumers
  • Act and interact to build cohesiveness
  • Assist with building a positive rapport with other units and community agencies

  • Demonstrate professional work ethics, as evidenced by overall dependability,

conscientiousness, and timeliness.

  • Other duties as assigned

PHYSICAL DEMANDS

  • While performing the duties of this job, the employee is regularly required to perform
  • physical activities, drive, stand and walk. In addition, the employee may be required to
  • restrain a client or physically intervene to resolve an altercation. The physical demands
  • described must be met by an employee to successfully perform the essential functions
